By registering for this course:

I understand that travel to and from the Paint class on the University of South Alabama
campus is my own responsibility. I understand that participation in this class is voluntary and not
required by the Center for Continuing Education or the University of South Alabama. I acknowledge that I have had the opportunity to ask questions to my satisfaction regarding this class prior to registering for this class.

In consideration of the Center for Continuing Education and the University of South Alabama allowing me to participate in this class, I, in full recognition and appreciation of any and all risks, hazards or dangers inherent in this class, including transportation to and from the University of South Alabama, to which I may be exposed, do hereby agree to assume all of the risks and responsibilities surrounding participation in such class. I understand that the Center for Continuing Education and the University of South Alabama, its trustees, officers, agents, servants and employees assume and accept no liability for personal injury or loss of life or damage to personal property.

Further, I, for myself and my heirs and personal representatives, do hereby defend, hold harmless and
indemnify, release and forever discharge the Center for Continuing Education and the University of South Alabama, its trustees, officers, agents, servants and employees from and against any and all claims, demands and actions or causes of action on account of or resulting from participation in the aforesaid class. I understand that I AM NOT covered by any University liability insurance.
